PROBLEM TO BE SOLVED: To provide a particle beam irradiation apparatus capable of accurately forming dose distribution in a desired depth direction, and to provide a particle beam irradiation method.SOLUTION: A particle beam irradiation apparatus includes: an accelerator accelerating charged particle beam a rotary irradiation body which transports the accelerated charged particle beam and rotates around an irradiation target to set an irradiation direction of the charged particle beam to the irradiation target into a plurality of directions and a range adjustment device which has a filter having a plurality of parallel ridge-like structures and allowing the transported charged particle beam to pass therethrough to enlarge the dispersion of the momentum of the charged particle beam, adjusts a range of the charged particle beam at the irradiation target and irradiates the irradiation target with the charged particle beam from the set irradiation direction in the adjusted range.
