PROBLEM TO BE SOLVED: To provide a treatment instrument for a basket type endoscope in which insertion resistance of a flexible sheath is small and insertion into a body cavity is easy. SOLUTION: A basket portion 13 for elastically returning to a bent state outward to widen the wire interval and taking in foreign matter inside when pushed forward from the distal end portion of the flexible sheath, and a flexible sheath. A guide wire engaging means for guiding along the guide wire 14 while maintaining the state inside the distal end portion of the first tip, and the first tip is provided with a first groove portion recessed to have a required depth from the tip end surface. A second groove portion and a third groove portion that are shallowly provided, and the second groove portion and the third groove portion are provided on a straight line intersecting with the first groove portion, and are guided in the first groove portion. A part of the guide wire engagement ring 26 for inserting the wire is accommodated, and the guide wire engagement bar 27 is accommodated in the second groove portion and the third groove portion while pressing the guide wire engagement ring.
