The present invention relates to a multi-tier secure telemedicine and e-healthcare system named iMediX, adapted to providing a low cost solution for teleconsultation, telemedicine and management OF chronic diseases by electronic health record (EHR) system. The system comprises a four tier telemedicine system comprising a Web Proxy Tier in between Presentation tier and Client and residing in the Demilitarized Zone, an application layer comprising of said presentation tier and a - core logic tier and a Database tier. Advantageously also, Database tier and the said Application layer are placed behind the firewall to restrict direct access from the Client. The iMediX system installed in a server is accessed by client machines through internet connection using standard internet browsers. The telemedicine and Electronic Health Record(EHR) System favour management of any diseases with adaptability/ desired interface to treat chronic diseases, particularly pediatric population suffering from HIV, Tuberculosis and the like from a distant location in a secured and reliable manner.
