Guide device for cutting and aligning bone pieces comprising a semi-cylindrical body with a C-section, whose internal face is the negative of the surface of a portion of bone in the intervention zone, and which is adjusted to the bone by an auto- attachable and pressure fixing or "Snap Fit". The custom self-attachable guide defines a groove to guide a blade in each cutting plane. The groove is oriented at a certain preselected angle and in a position relative to two anteroposterior and transverse anatomical axes of the patient, when coupled to the corresponding portion of the bone to be intervened. The position and angle are selected during pre-surgical planning, it also includes notches or signals to determine the alignment in the sagittal axis or the rotation of the post-osteotomy bone ends.
