PROBLEM TO BE SOLVED: To provide a medical image processing device, a treatment system, a medical image processing method, and a medical image processing program capable of improving reliability.SOLUTION: A medical image processing device includes an acquisition unit, a first feature site detection unit, and an estimation unit. The acquisition unit acquires a transparent image of a subject imaged by an imaging device. The first feature site detection unit detects the position of a feature site in the transparent image acquired by the acquisition unit.
