The present invention relates to an AI-based surgery assistant system for operating the lesion of a bone. The AI-based surgery assistant system comprises: a jig device disposed at the surgical site of a patient and having an ultrasound probe for capturing an ultrasound image； and a main device having an image storage unit which stores a three-dimensional medical image of the patient, and a surgical assistance planning module which establishes a surgical assistance plan by using the ultrasound image transmitted from the jig device. The surgical assistance planning module includes: a bone ultrasound modeling module generating a three-dimensional bone ultrasound image by using the ultrasound image； a bone lesion modeling module learning a three-dimensional medical image stored in the image storage unit based on deep learning to generate a three-dimensional bone lesion image in which the location of a bone lesion is displayed； an image matching module matching the bone ultrasound image and the bone lesion image to generate a three-dimensional bone lesion matching image； and a point position determination unit transmitting, to the jig device, surgical point position information input on the basis of the position in the bone lesion matching image.
