The present invention provides a manufacturing method of extract of salted and fermented shrimp, which comprises a step of naturally fermenting salted and fermented shrimp in a cave, a step of freezing and drying the fermented shrimp and powderizing, and a step of extracting the powdered fermented shrimp by adding alcohol; extract of salted and fermented shrimp manufactured thereby; a pharmaceutical composition including the extract of salted and fermented shrimp; and a health food composition. The extract of salted and fermented shrimp according to the manufacturing method of extract of salted and fermented shrimp of the present invention is excellent in the cholesterol decrease and the anti-fatty liver effect, and is applicable to a pharmaceutical composition and a health food composition, which provides a technology capable of increasing the nutritional value and contributing to the industry of salted and fermented shrimp.
