The present invention concerns a high-speed rotational atherectomy device for opening a stenosis in an artery having a given diameter, comprising:a guide wire (15) having a maximum diameter less than the diameter of the arterya flexible, elongated, rotatable drive shaft (20) advanceable over the guide wire and having a rotational axis.a concentric abrasive element (121C) disposed on the drive shaft (20) and having a center of mass located on the rotational axis of the drive shaftan eccentric distal counterweight (124E) disposed on the drive shaft (20) spaced apart distally from the concentric abrasive element (121C) by a distal spacing and having a center of mass spaced radially away from the rotational axis of the drive shaft andan eccentric proximal counterweight (123E) disposed on the drive shaft (20) spaced apart proximally from the concentric abrasive element (121C) by a proximal spacing and having a center of mass spaced radially away from the rotational axis of the drive shaft.
