FIELD: medicine.SUBSTANCE: invention relates to medicine, namely to cardiovascular surgery. In the preoperative period, multispiral computed tomography (MSCT) of chest organs is used to assess the manifestation of a commissural process, proximity of the right ventricle to the posterior surface of the sternum. Duplex peripheral vascular scanning is performed to rule out the presence of arteriosclerotic arteries. Then, after three 5-mm punctures on thorax in 3, 4 intercostal spaces along middle-clavicular and 5 intercostals space along anterior axillary line on the right or on the left - depending on the results of previous MSCT, an endovideo camera and working tools are inserted into the pleural cavity. Total adhesiolysis of the anterior mediastenum is performed under complete visual control； the posterior surface of the sternum is completely free from the underlying structures. After performing the adhesiolysis in the retro-sterilized space, a lower severity is performed without risk of damage to the underlying structures.EFFECT: method enables providing complete elimination of the risk of damaging anterior mediastinum organs in resternotomy without increasing surgical access injuries accompanying adhesion； reduced IC time used in operations without an endovideososurgical approach for adhesion, thereby reducing invasiveness of the operation.1 cl, 3 exИзобретение относится к области медицины, а именно к сердечно-сосудистой хирургии.
