The present invention relates to a surgical guide for a dental implant procedure where an alignment reference is marked and a method for manufacturing the same, which are to allow the surgical guide for implant procedure guiding to be accurately disposed at a position in an oral cavity, the method including: a first step in which an oral scan image is obtained by oral scanning of the inside of the oral cavity of a procedure object, a CT image is obtained by CT imaging upper and lower jaws, and an alignment line is virtually placed along a profile connecting visible reference indices set in advance on facing outside and inside surfaces of a gum portion including a tooth implanting target portion marked in the oral scan image a second step in which a three-dimensional occlusion guide image is obtained by alignment based on comparison regions respectively matching in pair with the oral scan image and the CT image and a third step in which the surgical guide is manufactured based on the three-dimensional occlusion guide image, wherein the surgical guide includes in inner surface thereof a body portion having a fixed groove portion matching the internal shape of the oral cavity of the procedure object, and a guide hole formed along a fixture implanting position, and having an alignment mark portion at a position corresponding to the alignment line such that an installation position is guided in the oral cavity.임플란트 시술을 안내하는 서지컬 가이드가 구강 내부의 정확한 위치에 설치되도록, 본 발명은 피시술자의 구강 내부를 오랄스캔하여 오랄스캔 이미지를 획득하고, 상하악을 CT촬영하여 CT이미지를 획득하되, 상기 오랄스캔 이미지에 표시된 치아 식립대상부가 포함된 잇몸부의 상호 대향되는 외측면과 내측면에 기설정된 가시기준지표를 연결한 프로파일을 따라 정렬라인이 가상배치되는 제1단계 상기 오랄스캔 이미지와 상기 CT이미지에 각각 쌍으로 매칭되는 비교영역을 기준으로 정합하여 3차원 교합 가이드 이미지를 획득하는 제2단계 및 상기 3차원 교합 가이드 이미지를 기반으로 내면에 상기 피시술자의 구강 내부 형상에 형합되는 고정홈부가 형성된 몸체부 및 픽스츄어 식립 위치를 따라 형성된 가이드홀을 포함하되, 구강 내부에서 설치 위치를 가이드하도록 상기 정렬라인과 대응되는 위치에 정렬표식부가 형성된 서지컬 가이드를 제조하는 제3단계를 포함하는 정렬기준이 표시된 치아 임플란트 시술용 서지컬 가이드의 제조방법을 제공한다.
