A derivative of natural insulin or an insulin analogue for the treatment of a disease or condition in which administration of insulin will benefit, including administering to a patient in need thereof, effective doses of the derivative, wherein said insulin derivative shows a prolonged action profile and where the indicated doses are administered at intervals , where a) the average value of these intervals is less than 56 hours, and at least one of these intervals is i. at least 1.04 of the average of the indicated intervals, or ii. no more than 0.96 of the average value of these intervals; or b) at least one of these intervals has a duration i. at least 1.3 of the average of the indicated intervals, or ii. not more than 0.85 of the average value of the indicated intervals, where the specified derivative of the indicated natural insulin or the indicated insulin analogue has a side chain attached either to the α-amino group of the N-terminal amino acid residue of the B chain or to the ε-amino group of the Lys residue present in the parent insulin B chain, the side chain having the general formula: -WXYZ, where W is: - an α-amino acid residue with a carboxyl group in the side chain, which forms an amide group using one of its carboxyl groups together with the α-amino group of the N-terminal amino acid residue of the B-chain or together with the ε-amino group of the Lys residue present in the B-chain of the parental insulin; - a chain consisting of two, three or four α-amino acid residues linked to each other through amide bonds, which is linked via an amide bond to the α-amino group1.
