The sleeve member has aguide slot (136) thatis sized to receive axial passage of at least a portion of a guide (108). Theguide slot and/or sleevemember may be positioned and/or configured to facilitate at least linearand/or rotational displacementof the forming tool about, or relative to, the guide, and thereby provide adegree of freedom inthe location at which the forming tool may form a shape or opening in the bonerelative to one ormore reference axes. The handle member may include a connection member (214)that is structuredto be operably coupled to a bone preparation device (102) that is structuredto facilitate the displacementof bone material.
