Embodiments described herein relate to treating a subject with advanced-stage GvHD with a1-antitrypsin (AAT) or carboxyterminal peptide derivatives thereof or recombinant thereof. In certain embodiments, a subject can include a glucocorticoid-refractory patient having GvHD with gut involvement. In other embodiments, donor subjects of a transplant can be treated with a1-antitrypsin (AAT) or carboxyterminal peptide derivatives thereof or recombinant thereof to reduce graft rejection or GvhD in a recipient subject.
