The present creation provides a surgery device for high tibial osteotomy comprising a substrate, a guiding slot, a guiding side, and a plural of fixing slots. The substrate has a first surface and an edge. The guiding slot is disposed on the first surface for guiding a saw and limiting the saw on a first cutting position. The guiding side is disposed on the edge for guiding the saw on a second cutting position. The fixing slots are disposed on the first surface for fixing the surgery device on a surface of a tibial. Using the present creation, the saw will cut the tibial according to the first and the second cutting position for standardization of the high tibial osteotomy.
