Provided is a guide that is easy to design and that can perform corrective osteotomy safely and accurately. An osteotomy guide 2, a guide wire inserted into a deformed bone, and a correction guide 4 are provided. The osteotomy guide includes first and second cylindrical legs 7 and 9 through which a guide wire can be inserted, and can be separated into first and second portions 2a and 2b with a separation surface 10 as a boundary. The lower end surfaces of the first and second cylindrical legs contact a predetermined position on the deformed bone without a gap. When the osteotomy guide is disposed at a predetermined position on the deformed bone, the separation surface is located on the same plane as the cutting line on the bone. After the guide wire is inserted into the deformed bone, the first portion is separated and removed before cutting the deformed bone. The correction guide includes parallel first and second guide tubes 17 and 18 through which a corresponding guide wire of the divided deformed bone can be inserted. By inserting guide wires through the first and second guide cylinders and arranging them parallel to each other, the mutual positional relationship between the deformed bones after the division is corrected.
