A method and implant to treat anterior cruciate ligament (ACL) injuries are disclosed. The method involves performing the cylindrical osteotomy of the proximal tibia and rotating the tibia plateau to a position of about 90 degrees to the patellar tendon with the stifle extended. The grate-like implant of the same shape and the thickness as the saw used to perform the osteotomy is specifically designed to facilitate transfer of compressive loads between the proximal and distal tibia segments. The implant also comprises a flange that is fixed by screws to the bone segments to retain the position of the grate-like spacer within the osteotomy gap and also to maintain the relative position of the bone segments after the leveling of the tibia plateau.
