PROBLEM TO BE SOLVED: To provide an evidence collection sheet capable of reliably guarantee correlation between a collected evidence and related data.SOLUTION: An evidence collection sheet 1 includes a transparent sheet 2 whose reverse face is coated with an adhesive layer for collecting an evidence, and a mounting sheet 3 whose surface is stuck with the transparent sheet 2.
