A device or insole for an item of footwear with a plantar surface 3 for engagement of a user's sole below the arch and heel, a joint 2 in use located at the user's metatarsal and phalange joints and an inclined end 1 arranged to engage the user's phalanges. The device is designed to lift the front part of a foot of a person affected by drop foot. The joint 2 deviates the surface into the end. The device is arranged to be worn in an item of footwear and the joint is arranged to deviate the item of footwear. The device may have footwear securement means which could comprise adhesive or hook and loop portions. The device may also have a textured surface 33, a toe cover, an adjustable joint, support below the end or a foot attachment mechanism, possibly comprising elastomer.
