PROBLEM TO BE SOLVED: To provide a hyperuricemic agent composition by which a renal function can be maintained through administration of a therapeutically effective amount of a xanthine oxidoreductase inhibitory compound or salt thereof.SOLUTION: A pharmaceutical composition contains 2-[3-cyano-4-(2-methyl propoxy)phenyl]-4-methylthiazole-5-carboxylic acid (general name: febuxostat) or the like, or pharmaceutically acceptable salt thereof by a therapeutically effective amount, the subject is affected by gout, acute gout arthritis, chronic gout articular disease, or tophaceous gout, a renal function is evaluated at least by one of a serum creatinine level, a creatinine clearance level, and a glomerular filtration rate (GFR), GFR of the subject is maintained at a level approximately 75% or higher when compared to a GFR level being a baseline of the subject, a serum uric acid value of the subject is maintained lower than the level of 2.6 to 7.2 mg/dL or at this level, and the composition is administered to the subject for a period longer than 30 months.SELECTED DRAWING: Figure 2【課題】キサンチン酸化還元酵素阻害化合物又はその塩の治療有効量の投与によって腎機能が保持できる抗高尿酸血性剤組成物の提供。【解決手段】２－［３－シアノ－４－（２－メチルプロポキシ）フェニル］－４－メチルチアゾール－５－カルボン酸（一般名：フェブキソスタット）等又は医薬として許容可能なその塩を治療有効量含み；前記対象が、－痛風、急性痛風関節炎、慢性痛風関節疾患又は結節性痛風に罹患しており、－腎機能が、血清クレアチニンレベル、クレアチニンクリアランスレベル及び糸球体濾過量（ＧＦＲ）の少なくとも１つにより評価され、－前記対象のＧＦＲが、前記対象のベースラインであるＧＦＲレベルと比較した場合に約７５％以上のレベルに維持され、－前記対象の血清尿酸値が２．６～７．２ｍｇ／ｄＬのレベルよりも低いか又はこのレベルに維持され、且つ、３０ヶ月よりも長い期間に亘り前記対象に投与される医薬組成物。【選択図】図２
