An apparatus is provided for determining a change in a level of care for apatient(s).The apparatus includes at least one memory and at least one processorconfigured todetermine that at least one of an admission of a patient to a health carefacility, a discharge ofthe patient from the health care facility, or a transfer of the patient withinthe health carefacility triggered a level of care change event associated with the patient.The processor isalso configured to identify a care event(s) for the patient in response todetermining that thelevel of care change event was triggered. The processor is also configured togenerate anotification(s) to perform a task(s) associated with the care event(s) for thepatient andassociate the care event(s) and a task(s) with the triggered level of carechange event.Corresponding computer program products and methods are also provided.
