A catheter and tissue cutting system percutaneously permits the creation of an anastomosis between a first and second anatomical structure, such as a vein and an artery. The system comprises a catheter having a main body with a lumen and tapered distal tip, configured to be moved distally into the first anatomical structure over a primary guidewire. A cutting electrode is nested in the main body, with a lumen which tracks over a secondary guidewire, and is insertable into the secondary anatomical structure. An energy supply is operative to energize the cutting electrode in order to cut a tissue wall defining the first anatomical structure.La présente invention concerne un cathéter et un système de découpe de tissu permettant la création par voie percutanée d'une anastomose entre une première et une seconde structure anatomique, telle qu'une veine et une artère. Le système comprend un cathéter comportant un corps principal ayant une lumière et une pointe distale effilée, conçu pour être déplacé de manière distale dans la première structure anatomique sur un fil guide principal. Une électrode de découpe est emboîtée dans le corps principal, une lumière suivant un fil guide secondaire, et peut être insérée dans la structure anatomique secondaire. Une alimentation en énergie sert à exciter l'électrode de découpe afin de couper une paroi tissulaire délimitant la première structure anatomique.
