PROBLEM TO BE SOLVED: To easily acquire a distance between an endoscope tip and a specific point, and a distance between the endoscope tip and a region of interest in an endoscopic image diagnosis support device, system, method, and program.SOLUTION: An image acquisition section 71 acquires a real endoscopic image T0, and a virtual endoscopic image generation section 74 generates a virtual endoscopic image K0 corresponding to a point of view of the real endoscopic image T0. A projection image generation section 75 generates a first projection image R1 formed by projecting a region of interest onto an inner wall of the bronchi with a specific point set inside the bronchi defined as a reference, and a second projection image formed by projecting the region of interest onto the inner wall of the bronchi with a position of a tip 3B of the endoscope defined as a reference.
