Systems and methods are provided for adjusting a sustained insulin drug dosage in a subject. Multiple time-stamped glucose readings of the subject are obtained. The titration glucose level (246) is calculated as the central tendency (244, 268, 274) of the measures of the titration subset (240, 269, 273) of the small glucose readings identified within the time stamped glucose readings. The central tendency of the scale represents the scale of glucose measurements in the small primary time frame. The sustained insulin drug dose (216) is adjusted or maintained based on the titrated glucose level (246) obtained.
