The surgical operation guide image system according to the present invention includes: a transparent display which is formed in a glasses shape; an affected area image storage unit which photographs an affected area image of a patient by using at least one among a computed tomography (CT), a magnetic resonance imaging (MRI), and an X-ray and stores the photographed affected image of the patient in a database; an affected area image receiving unit which receives the affected area image of the patient from the affected area image storage unit; an image converting unit which converts the affected area image of the patient which is received through the patient image receiving unit into a transparent image; and an image combining unit combines and displays the images so that the affected area image of the patient which is converted into the transparent image by the image converting unit corresponds to the real image of the patient through the transparent display.
