A negative pressure suction-type bone cement injection device, comprising an inner cylinder (13), an intermediate cylinder (22), and an outer cylinder (23). The inner cylinder (13) is provided with a piston rod (11), and an inner needle tube (25) is mounted at a front end of the inner cylinder. A side wall of the intermediate cylinder (22) is provided with a suction pipe (221) connected to a vacuum pump device. A front end of the intermediate cylinder (22) is provided with an outer needle tube (24), the inner cylinder (13) is fixed inside the intermediate cylinder (22), and the inner needle tube (25) passes through the outer needle tube (24). The intermediate cylinder (22) is sleeved inside the outer cylinder (23) to form a movable structure. The device extracts air, bone powder, liquid blood, and blood clots at a bone site to be repaired by providing the suction pipe (221) on the side wall of the intermediate cylinder (22) in combination with a vacuum extraction device, so as to form a negative pressure in the bone cavity to suck bone cement into an injection device, such that pressure produced by pushing the piston rod (11) is prevented from causing the bone cement to seep out of a bone crack resulting in nerve compression and other adverse consequences. A manual pulling structure and a rotary driving structure are further utilized to facilitate causing the intermediate cylinder (22) to drive the inner cylinder (13) to move backwards, and a push rod (1) is not moved to realize bone cement injection. This supplements the negative pressure injection, and can also substitute the negative pressure injection when a negative pressure is unavailable.
