An adjustable segmental distalization appliance for orthodontic treatmentsfeatures elongated first and second components that cooperatively define acentral body of the appliance spanning longitudinally between an anterior endofthe first component and a distal end of the second component. First and secondterminal heads are respectively carried at the anterior and distal ends andadapted for attachment to first and second teeth of a dental arch. The firstandsecond components are arranged for selective longitudinal displacementrelativeto one another to adjust an overall length of the central body to adapt to apatient-specific mesial-distal distance between the first and second teeth ofthedental arch. In one embodiment, one component is a tubular female componentcarrying a ratchet pawl, and the other component is an insertable malecomponent with cooperating ratchet teeth for incremental extension of thecentral body.
