An instrument to create microfractures in subchondral bone without skiving and a method of using the same are provided. The instrument includes an outer sleeve having a handle coupled to a hollow tube, wherein the end of the tube is curved and includes features to secure the end on a subchondral bone. A puncture element is provided and designed to be inserted through the outer sleeve and translate and/or rotate therethrough into the subchondral bone when directed. The end of the puncture element having a surface that allows the tip to penetrate into the bone to provide a microfracture therethrough without the instrument skiving across the subchondral bone.Linvention concerne un instrument pour créer des micro-fractures dans un os sous chondral sans asservissement et un procédé dutilisation dudit instrument.
