PROBLEM TO BE SOLVED: To reduce friction between a wire and a pulley.

SOLUTION: The medical manipulator 10 includes a wire 56a movable in opposite directions, a driven wire 252a having both ends thereof connected to the wire 56a, and a guide pulley 142a formed of two layers of a first layer guide pulley 236a and a second layer guide pulley 238a, a driven pulley 156, and a return pulley 350 in this order from the proximal end side. The driven wire 252a passes a first side of the first layer guide pulley 236a to be trained around a first side of the driven pulley 156 toward a second side, and is disposed on a second side of the second layer guide pulley 238a. The driven pulley 156 is mounted obliquely, so that the first side of the first layer guide pulley 236a and the first side of the driven pulley 156 are parallel to each other, and that a second side of the driven pulley 156 and the second side of the second layer guide pulley 238a are parallel to each other.
