System for guiding the manual insertion of a needle (1) into the body (2) of a patient during a percutaneous surgical procedure, having a guiding device (5), which has a display (6) and markers (7a-7d) able to reflect the infrared radiation and can be mounted on the needle (1) with the markers (7a-7d) arranged according to a geometrical pattern, infrared video cameras (9) for acquiring images of the guiding device (5) during the surgical procedure, and a computer (10), which has a memory (11) for storing the positions of an entry point (PE) and a target point (PT) of the needle (1) in the body (2) said system is interfaced with the video cameras (9) and the display (6) and is configured to determine, in real time, the position of the markers (7a-7d) on the basis of the images acquired and the current position of the needle(1) as a function of the positions of the markers (7a-7d) and the geometrical pattern, and to control the display (6) to show graphical navigation information which is a function of the position of the needle (1) and of the entry and target points (PE, PT).
