PROBLEM TO BE SOLVED: To provide an ophthalmic surgical microscope capable of securing a favorable illumination state for patients eyes during surgery and especially, illuminating (red reflection) a crystalline lens from the back plane, and allowing an operator to perform a cataract operation and the like under favorable illumination conditions.SOLUTION: The ophthalmic surgical microscope comprises: an illumination optical system for applying illumination light emitted from an illumination light source to a patients eye and an observation optical system for observing the patients eye illuminated by the illumination light.
