A guiding device for minimally invasive pedicle screw insertion, comprises: two positioning rods for being fixed on a vertebra to provide two non-parallel rod bodies a positioning plate having a first positioning tube body, a second positioning tube body, an elongated lateral notch, and a plurality of sleeve assemblies, wherein the second positioning tube body has a longitudinal opening to communicate with the elongated lateral notch. Therefore, in the process of inserting one rod body of the two non-parallel rod bodies through the first positioning tube body, the other rod body may enter the second positioning tube body through the elongated lateral notch and the longitudinal opening, thereby completing a positioning procedure. The guiding device further includes a plurality of metal nails which are used after the positioning procedure is completed, wherein the plurality of metal nails are inserted into the vertebrae through the plurality of sleeve assemblies.
