PROBLEM TO BE SOLVED: To provide an endoscope system, processor device, and operation method for the endoscope system for extracting a blood vessel of a patient.SOLUTION: An endoscope system 10 includes: an endoscope 12 a light source device 14 and a processor device 16. The endoscope 12 images a patient and outputs an image signal. The light source device 14 illuminates the patient. The processor device 16 has an image processing unit for highlighting blood vessel 60. The image processing unit for highlighting blood vessel 60 includes: an image generating unit 70, a first blood vessel extraction part 72 a second blood vessel extraction part 74 and a composition processor 76. The image generating unit 70 generates an endoscopic image based on the image signal. The first blood vessel extraction part 72 outputs a first blood vessel extraction image including a middle-deep layer blood vessel extracted from the endoscopic image. The second blood vessel extraction part 74 outputs a second blood vessel extraction image including the middle-deep layer blood vessel and surface layer blood vessel extracted from the endoscopic image.
