To provide an individual identification tag which is light and a little burden to a poult and in which an individual identification information label can be easily attached to a position where the attached label is easily recognized optically.SOLUTION: An individual identification tag 1 is provided on which individual identification information is displayed, and which is attached to an animal. The individual identification tag 1 includes: a first curved plate 11 and first flat plate 12 which are formed so as to be continuous with each other； and a second curved plate 21 and a second flat plate 22 which are formed so as to be continuous with each other. The first curved plate and the second curved plate are connected to each other using a hinge 3. A lock part 13 and a needle part 23 are provided on a tip of the first flat plate and on a tip of the second flat plate, respectively. The needle part protrudes in a hanging condition downwards from an inner circumferential surface of the second flat plate and has: a tip 26 which is acuate in an approximately triangular shape； a serrated fitting part 27 in the vicinity of the tip； and a falling prevention branch 28 protruding obliquely upwards from a side surface of the needle part. The lock part has a serrated step part 17 inside a slit 19 into which the tip of the needle part is inserted.
