PROBLEM TO BE SOLVED: To provide an apparatus for preparing a drug which prevents only medical liquid in a prefilled syringe from being administered and facilitates the preparing work of the syringe that sucks a prepared drug.SOLUTION: The apparatus 1 for preparing the drug includes the prefilled syringe 3 including an outer cylinder 10 and a cap 30, a drug container 4, and an adapter 2. The adapter includes an adapter body 20 and a puncture appliance 7 which is movable in the adapter body. The puncture appliance includes a mounting part capable of mounting the cap of the syringe, an engaging part 73a at the puncture appliance side, and a hollow needle 71.
