Disclosed are a patient-customized 3D bone cutting guide, and a manufacturing method, a recording medium, and a manufacturing apparatus thereof. According to one embodiment of the present invention, a bone cutting guide manufacturing method comprises: a) an image file generation step of generating a 3D skull image file of a patient; b) a symmetry determination step of using the 3D skull image file to determine whether the mandibular is symmetric based on the left and right central planes of the face and the mandibular; c) a mirroring step of mirroring a normal portion from the left and right sides of the skull based on to the left and right central planes of the face and the mandibular to overlap an image representing the normal portion with an image representing an abnormal portion based on data acquired through the symmetry determination step; d) a cut portion extraction step of extracting a bone portion to be cut from the abnormal portion through the mirroring step; e) a shape data extraction step of extracting shape data of a guide main body with a structure corresponding to a surface structure of the bone to be cut and drill hole shape data considering cut depth data of the bone to be removed based on the data acquired through the cut portion extraction step; and f) a bone cutting guide manufacturing step of manufacturing a bone cutting guide including the guide main body with a drill hole formed therein based on the data acquired through the shape data extraction step.
