PROBLEM TO BE SOLVED: To obtain a medical image forming apparatus capable of forming an accurate two-dimensional visible image of luminal tissue by excluding the effect of peripheral tissue, a medical image forming program, and a medical image forming method.SOLUTION: The medical image forming apparatus 1 is constituted so that the three-dimensional medical image data of a noticeable blood vessel is extracted in a predetermined extraction width from three-dimensional medical image data acquired by an image forming apparatus 4 and the extracted three-dimensional medical image data of the noticeable blood vessel is projected on a two-dimensional plane in a three-dimensional space to form the two-dimensional visible image of the noticeable blood vessel.
