A medical implant is described herein. In one embodiment, the implant includesasubstrate having at least one surface. The implant has a coating positioned onat leasta part of the surface. The coating includes a bioactive material and anantimicrobialagent. The concentration of the antimicrobial agent varies with distance fromasubstrate surface-coating interface. The coating includes a number of layers,and theconcentration of antimicrobial agent is different in at least two coatinglayers. Theconcentration of antimicrobial agent is greater further from the substratesurface-coatinginterface than nearer to the substrate surface-coating interface. Theantimicrobial agent includes metal and/or metal ions.
