Devices, systems and methods are disclosed for the formation of an arteriovenousfistula in the limb of the patient. Embodiments include a device (10) for the creation,modification and maintenance of a fistula that includes an integral fistulacreation assembly (25) near its distal end that passes through the skin of thepatient, through a first vessel such as an artery, and into a second vessel suchas a vein. The fistula creation assembly (25) preferably includes an anastomoticimplant (60) that is placed within the fistula t maintain long term blood flowthere through. The devices, systems and methods can be used to treat patientswith one or more numerous ailments including chronic obstructive pulmonarydisease, congestive heart failure, hypertension, hypotension, respiratorfailure, pulmonary arterial hypertension, lung fibrosis and adult respiratorydistress syndrome.
