PROBLEM TO BE SOLVED: To provide a medical actuator which can alter, with high accuracy, the attitude of a tool provided at the tip of an elongated pipe part and can suppress friction of mutual contact places without having an adverse effect on a living body.SOLUTION: The medical actuator includes a spindle guide part 3 in an elongated shape, a tip member 2 attached to the tip of the spindle guide part so that the attitude thereof can be altered, and the tool 1 provided rotatably at the tip member 2. The tip member 2 rotatably supports a spindle 13 holding the tool 1. The spindle guide part 3 internally has a rotary shaft 22 transmitting rotation to the spindle 13 and a guide hole 30a penetrating through both ends. An attitude operation member 31 inserted inside the guide hole 30a so that it is movable forward and backward is made to move forward and backward so as to alter the attitude of the tip member 2.
