A device for facilitating treatment via a vascular wall defining a vascular lumen containing an occlusion therein, the device comprising a crossing device (320) comprising a tip (324) fixed to a distal end of a shaft (322) and a hub (346) fixed to a proximal end of the shaft (322), the shaft including a coil (334) wound in a helical shape and extending from the distal end of the shaft (322) to the proximal end of the shaft (322), a polymer sleeve (336) extending from the distal end of the shaft (322) and disposed about a portion of the coil (334) and a metallic tubular body (338) and a handle assembly (350) positioned about the crossing device (320) and configured to be coupled to the crossing device (320) such that torque is applied to the shaft (322) when the handle assembly (350) is rotated.
