PROBLEM TO BE SOLVED: To provide a sustained release solid pharmaceutical preparation controlling the elution of an active ingredient to the zero-order elution without being influenced with in vivo environments or compression molding force with slight variation of gastrointestinal tract retention time.SOLUTION: The sustained release solid pharmaceutical preparation comprises one or more kinds of active ingredients and an elution-controlling base. Furthermore, the elution-controlling base comprises (a) a modified starch having &ge400% water-holding capacity, &ge200 g gel push-in load, 40-95 wt.% content of a water-soluble ingredient, &ge90 wt.% of particles passing through a sieve having 75 &mum opening, &ge20 wt.% of particles passing through a sieve having 32 &mum opening and &ge20 to <50 &mum average particle diameter and (b) a hydrophobic adjuvant having &ge8 h disintegration time, &ge2 kg push-in strength and &le3 mL sedimentation volume when dispersed in water.
