An ultrasound catheter (1() and system (10) includes a catheter sheath (24) having a proximal end (24-1), a distal end (24-2), and a lumen (26) extending between the proximal end (24-1) and the distal end (24-2) in a longitudinal direction. A core wire (20) is movably disposed within the lumen (26) of the catheter sheath (24). The core wire (20) has a proximal portion (20-3) and a distal head portion (30). The proximal portion (20-3) of the core wire (20) is coupled to an ultrasound vibration source (14) configured to transfer the vibration motion to the core wire (20). The core wire (20) is configured to move relative to the catheter sheath (24) to selectively extend and retract the distal head portion (30) of the core wire (20) relative to the distal end (24-2) of the catheter sheath (24) in the longitudinal direction.
