The present invention provides the following: a cardiovascular disease primary prevention agent, which comprises as the active ingredient at least one selected from the group consisting of EPA, salts thereof, and esters thereof and is for lowering the risk of cardiovascular disease by administration to subjects having a blood (serum or plasma) hs-CRP level of 1.0 mg/L or higher in spite of no history of cardiovascular disease； a combination marker comprising the blood hs-CRP value and serum EPA/AA ratio for evaluating the primary risk of cardiovascular disease in subjects having no history of cardiovascular disease； and a method for extracting subjects with high risk of cardiovascular disease and/or a method for preventing cardiovascular disease.본 발명은 심혈관 질병의 기왕이 없고, 혈중(혈청 중, 혈장 중) hs-CRP값이 1.0㎎/ℓ 이상인 피험자에게 투여하고, 심혈관 질병 리스크를 저하시키기 위한, EPA, 그 염 및 에스테르로 이루어지는 그룹에서 선택되는 적어도 하나를 유효성분으로 함유하는 심혈관 질병 1차 예방제, 혈중 hs-CRP값과, 혈청 EPA/AA비를 포함하는 심혈관 질병의 기왕이 없는 피험자의 심혈관 질병 1차 리스크를 평가하기 위한 조합 마커, 심혈관 질병 리스크가 높은 피험자의 추출방법 및/또는 심혈관 질병 예방방법을 제공한다.
