PROBLEM TO BE SOLVED: To provide a band form having a large band and a small band that are in a state of not separated from each other, where the large band and the small band are not arranged in parallel.SOLUTION: A band form 100 includes a large band 40 and a small band 50 having different surface areas, where each large band 40 and small band 50 includes a printing part 41 (51), a strap part 42 (52) arranged adjacent to one end of the printing part 41 (51), and an installation part 43 (53) arranged at the other end facing the one end of the printing part 41 (51). The installation part 43 of the large band 40 is communicated with the strap part 52 of the small band 50. The large band 40 and the small band 50 are integrated to constitute a large and small band unit U.
