Device for remote monitoring the health state of a patient by a health care professional by means of audio and video communications means. The device is provided with a supervision unit, an internet connection means, sensors and respective printed circuit boards for monitoring the physiologic parameters of the patient, equipments for therapy administration to the patient and a PLC able to control the printed circuit boards and the equipments for the therapy administration. The supervision unit and the PLC allow to perform simultaneously the activation or deactivation operations of said equipments for therapy administration to the patient, to remotely monitor in real time the physiological parameters of the patient sensed by the sensors, and to perform video-calls. The data sensed can be used to evaluate instantly the efficacy of the therapy administration.
