A rotational thrombectomy wire (30, 60, 130, 130, 260, 360) for breaking up vascular thrombus or other obstructive material having a core (62) having a proximal portion and a distal portion. The distal portion has a smaller diameter than the proximal portion. A cable (80) extends distally of the core (62). The cable (80) has a cable covering material (88, 89) positioned external thereof. A first coil (90) is attached to a distal portion of the cable (80) and has a diameter larger than a diameter of the cable and has a coil covering material (87) positioned thereover. The wire (30, 60, 130, 130, 260, 360) is rotatable by a motor (52, 152, 152).
