To provide an improvement device having an endoscope and provide an instrument guided by the endoscope.SOLUTION: A device of this invention includes an endoscope shaft. A working channel 20 terminates on an outlet surface by penetrating the shaft. A guide element extends by penetrating the working channel and is guided in the longitudinal direction along the working channel in the working channel. The device is so configured that distal end sections 32a-c extending in the longitudinal direction adjacently to the shaft may be moved across an outlet surface along the shaft in order to move work sections 34a-c apart from the outlet surface.
