The present invention relates to a patient-customized surgical instrument, and more particularly, to a patient-customized surgical instrument for use in forming an implant for implantation in a femur or tibia, comprising: a body surrounding a portion of the femur or tibia； a support portion formed at a portion of the body which is in contact with the femur or tibia in a negative shape complementary to an anatomical shape of the femur or tibia； and a through hole formed in the body so that a drill for forming a drill hole can be inserted into the femur or tibia.
