A hip resurfacing CAS system for guiding anoperator in altering a femoral head comprises a trackablereference on the femur, a registration tool is trackable,a bone-altering tool associated with a resurfacing of thefemoral head, a tracking apparatus , and a resurfacingprocessing unit connected to the tracking apparatus soas to receive tracking data for the tools. Theresurfacing processing unit has a position/orientationcalculator to calculate from the tracking data a trackingof the femoral frame of reference, of the registrationtool and the bone-altering tool. A modelgeneratorreceives produces a model of the femoral head and neckwith respect to the femoral frame of reference. Aresurfacing evaluator determines an evaluated boneresurfacing alteration as a function of a tracking of thebone-altering tool with respect to the bone model of thefemoral head and neck, and a tool geometry model.
