The method involves laparoscopically forming a first entry through an abdomen. An applicator assembly is introduced through a second entry, where the applicator assembly is provided with a flexible introducer sleeve and a curved applicator tool (10) placed in the sleeve. A proximal end of the curved applicator tool is manipulated to position a distal end of the curved applicator tool at an identified exposed nerve. An electrode lead is placed through the applicator assembly to the nerve. The tool is laparoscopically observed through the first entry during manipulation of the proximal end. Independent claims are also included for the following: (1) an apparatus for implanting an electrode to an endopelvic portion of a pelvic nerve (2) an electrode assembly.
